Confira os principais pontos a serem considerados"},"content":{"rendered":"<p>A presen\u00e7a de defici\u00eancias de <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/micronutrientes\/\">micronutrientes<\/a>, sobretudo de alguns oligoelementos, pode prejudicar uma s\u00e9rie de fun\u00e7\u00f5es do organismo. Muitos processos org\u00e2nicos dependem de quantidades adequadas deles, bem como da combina\u00e7\u00e3o de tais elementos<sup>1<\/sup>.<\/p>\n<p>Dessa forma, a introdu\u00e7\u00e3o dos micronutrientes na nutri\u00e7\u00e3o parenteral ou em administra\u00e7\u00e3o endovenosa isolada pode ser essencial para prevenir ou corrigir esses estados de defici\u00eancia<sup>2<\/sup><strong>. \u00a0Isso tende a ser indispens\u00e1vel para manter o metabolismo normal dos macronutrientes e <\/strong><a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/antioxidantes-nutricao-parenteral\/\"><strong>a defesa antioxidante<\/strong><\/a><strong>, al\u00e9m de contribuir na promo\u00e7\u00e3o da cicatriza\u00e7\u00e3o de feridas e auxiliar o sistema imunol\u00f3gico <sup>2,3,<\/sup>.<\/strong><\/p>\n<p><strong>Todavia, reconhecer a import\u00e2ncia de corrigir tais defici\u00eancias n\u00e3o impede que haja alguns obst\u00e1culos. O principal deles, talvez, seja a pr\u00f3pria identifica\u00e7\u00e3o da defici\u00eancia.<\/strong> Como aponta <a href=\"https:\/\/braspenjournal.org\/article\/10.37111\/braspenj.AE.2021.36.1.01\/pdf\/braspen-36-1-3.pdf\">o posicionamento da Braspen (Sociedade Brasileira de Nutri\u00e7\u00e3o Enteral e Parenteral) sobre o tema,<\/a> dosar e monitorar a concentra\u00e7\u00e3o de micronutrientes no organismo nem sempre \u00e9 uma tarefa f\u00e1cil<sup>1<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<h2>Em que situa\u00e7\u00f5es a administra\u00e7\u00e3o de micronutrientes pode ser recomendada?<\/h2>\n<p>O mesmo documento da Braspen sugere que indiv\u00edduos recebendo nutri\u00e7\u00e3o parenteral em UTI, ocasionalmente, devem receber multivitam\u00ednicos e oligoelementos diariamente para conter a defici\u00eancia de <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/importancia-dos-micronutrientes-nutricao-parenteral\/\">micronutrientes<\/a><sup>1<\/sup>. Adicionalmente, pacientes mais graves podem demandar doses maiores desses componentes, seja como parte da nutri\u00e7\u00e3o parenteral, seja de forma isolada<sup>4<\/sup>.<\/p>\n<p>Em pacientes que n\u00e3o est\u00e3o recebendo nutri\u00e7\u00e3o parenteral, certas condi\u00e7\u00f5es podem exigir uma reposi\u00e7\u00e3o de maneira mais imediata. Ou seja, nesses muitos casos, a administra\u00e7\u00e3o endovenosa isolada de microelementos se faz necess\u00e1ria.<\/p>\n<p>Por isso \u00e9 que <strong>os profissionais respons\u00e1veis pelo acompanhamento de pacientes em diferentes circunst\u00e2ncias (incluindo unidades de terapia intensiva, por exemplo) devem estar cientes de que v\u00e1rias situa\u00e7\u00f5es tornam mais comum a defici\u00eancias de micronutrientes<sup>1<\/sup>.<\/strong><\/p>\n<p>Entre exemplos disso est\u00e3o situa\u00e7\u00f5es que levam \u00e0 perda excessivas desses elementos. Isso \u00e9 frequente em pacientes com grandes queimaduras, com f\u00edstulas digestivas, drenos e estomias de alto d\u00e9bito ou ainda naqueles submetidos \u00e0 <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/sarcopenia-pacientes-hemodialise\/\">hemodi\u00e1lise<\/a><u> cont\u00ednua<\/u><sup>1<\/sup>. Ao mesmo tempo, outras circunst\u00e2ncias podem provocar aumento da demanda por tais elementos. \u00c9 o caso de pacientes com traumas de grande porte<sup>1,4<\/sup>.<\/p>\n<p>Por fim, alguns quadros podem alterar a absor\u00e7\u00e3o dos micronutrientes. Entre as situa\u00e7\u00f5es que ilustram isso est\u00e3o ressec\u00e7\u00f5es do trato gastrointestinal, desvios do tr\u00e2nsito ou doen\u00e7as inflamat\u00f3rias intestinais<sup>2,4<\/sup> (para mais informa\u00e7\u00f5es, consultar infogr\u00e1fico).<\/p>\n<p>Ainda assim, \u00e9 sempre importante ressaltar que identificar as defici\u00eancias e necessidades de micronutrientes \u00e9 uma tarefa exigente<sup>1<\/sup>. <strong>Muitas das t\u00e9cnicas laboratoriais utilizadas para tal fim s\u00e3o caras, complexas e com resultados demorados. <\/strong>Al\u00e9m disso, alternativas mais acess\u00edveis, nem sempre refletem o estado real de determinado micronutriente no organismo<sup>1<\/sup><strong>.<\/strong><\/p>\n<p>Junto a tudo isso, \u00e9 preciso sempre ter em mente que as vari\u00e1veis de cada situa\u00e7\u00e3o podem exigir diferentes aportes na promo\u00e7\u00e3o da manuten\u00e7\u00e3o da boa condi\u00e7\u00e3o nutricional do paciente.<\/p>\n<p>&nbsp;<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-1980\" src=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/08\/lam163.png\" alt=\"\" width=\"940\" height=\"678\" srcset=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/08\/lam163.png 940w, https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/08\/lam163-300x216.png 300w, https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/08\/lam163-768x554.png 768w, https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/08\/lam163-520x375.png 520w\" sizes=\"auto, (max-width: 940px) 100vw, 940px\" \/><\/p>\n<p>&nbsp;<\/p>\n<p><em>Saiba mais: <\/em><a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/importancia-oligoelementos-nutricao-parenteral\/\"><em>A import\u00e2ncia dos oligoelementos na nutri\u00e7\u00e3o parenteral<\/em><\/a><\/p>\n<p>&nbsp;<\/p>\n<h2>De que forma situa\u00e7\u00f5es espec\u00edficas podem levar \u00e0 defici\u00eancia de micronutrientes?<\/h2>\n<p>Um exemplo dessas vari\u00e1veis s\u00e3o as condi\u00e7\u00f5es nutricionais de pacientes que passam por cirurgias do trato gastrointestinal (TGI). <strong>Como cada nutriente \u00e9 absorvido em determinado trecho do TGI, a ressec\u00e7\u00e3o ou a preserva\u00e7\u00e3o de cada um dos segmentos pode provocar d\u00e9ficits de elementos espec\u00edficos<sup>1<\/sup>. <\/strong>Essas altera\u00e7\u00f5es podem ser identificadas pela equipe multiprofissional de terapia nutricional respons\u00e1vel pelo acompanhamento do paciente cir\u00fargico.<\/p>\n<p>Como exemplo disso, sabe-se que a ressec\u00e7\u00e3o do duodeno e do jejuno proximal tem o potencial de provocar defici\u00eancias de oligoelementos como o <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/zinco-importancia-em-nutricao-parenteral\/\">zinco <\/a>e o cobre<sup>5<\/sup>. Enquanto isso, a ressec\u00e7\u00e3o g\u00e1strica prejudica a absor\u00e7\u00e3o do ferro, cobre e de uma s\u00e9rie de vitaminas<sup>5<\/sup>.<\/p>\n<p>Outro fator espec\u00edfico capaz de influenciar na defici\u00eancia de micronutrientes no paciente cir\u00fargico submetido a interven\u00e7\u00f5es no TGI \u00e9 a resposta end\u00f3crina metab\u00f3lica ao trauma<sup>1<\/sup>.<\/p>\n<p>Por conta disso, pode haver uma redistribui\u00e7\u00e3o dos micronutrientes devido \u00e0 influ\u00eancia das citocinas pr\u00f3-inflamat\u00f3rias (normalmente em vig\u00eancia de um PCR elevado)<sup>6,7<\/sup>. \u00a0Complica\u00e7\u00f5es decorrentes do procedimento, como f\u00edstulas, v\u00f4mito e diarreia podem tamb\u00e9m acelerar a defici\u00eancia de nutrientes espec\u00edficos (como sel\u00eanio e <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/ferro-na-nutricao-parenteral-seguro-e-necessario\/\">ferro<\/a>)<sup>8,9,10 <\/sup>.<\/p>\n<p>Como j\u00e1 ressaltado, v\u00e1rias consequ\u00eancias est\u00e3o atreladas \u00e0s defici\u00eancias de micronutrientes. <strong>Entre elas est\u00e3o a m\u00e1 cicatriza\u00e7\u00e3o, sinais de fraqueza muscular, resposta imunol\u00f3gica inadequada e outras disfun\u00e7\u00f5es org\u00e2nicas<sup>1<\/sup>.<\/strong><\/p>\n<p>&nbsp;<\/p>\n<h2>Que armadilhas podem estar escondidas em exames para a rastrear defici\u00eancias de micronutrientes?<\/h2>\n<p>A utiliza\u00e7\u00e3o de testes sangu\u00edneos costuma ser um recurso bastante empregado para avaliar a quantidade de micronutrientes no organismo em nossa pr\u00e1tica cl\u00ednica<sup>11<\/sup>. <strong>No entanto, o emprego dessas t\u00e9cnicas pode esconder algumas armadilhas capazes de confundir os profissionais diante dos resultados encontrados, <\/strong>inclusive quando s\u00e3o comparados resultados de diferentes laborat\u00f3rios<sup>11<\/sup>.<\/p>\n<p>\u00c9 preciso considerar, por exemplo, que para alguns oligoelementos (como o sel\u00eanio), <strong>tanto a an\u00e1lise do plasma e do soro sangu\u00edneo reflete mudan\u00e7as recentes na ingest\u00e3o alimentar<sup>11<\/sup><\/strong>. Contudo, medi\u00e7\u00f5es de sangue total e de contagem de gl\u00f3bulos vermelhos podem apontar uma trajet\u00f3ria de longo prazo com mais precis\u00e3o, com menor influ\u00eancia da alimenta\u00e7\u00e3o recente<sup> 12,13<\/sup>.<\/p>\n<p>De forma paralela, para determinados micronutrientes a an\u00e1lise da contagem de gl\u00f3bulos vermelhos no plasma ou soro demora mais para responder \u00e0 interven\u00e7\u00e3o aplicada. Essa lacuna de tempo \u00e9 explicada por conta do per\u00edodo necess\u00e1rio para a s\u00edntese de novas c\u00e9lulas vermelhas<sup>11<\/sup>.<\/p>\n<p>Portanto, em algumas circunst\u00e2ncias, <strong>essas medi\u00e7\u00f5es a partir do plasma\/soro s\u00e3o de valor limitado.<\/strong> Nem sempre h\u00e1 concord\u00e2ncia entre a concentra\u00e7\u00e3o dos elementos no plasma e na forma intracelular<sup>11<\/sup>. Outro ponto importante que merece a aten\u00e7\u00e3o \u00e9 a forma como respostas metab\u00f3licas \u00e0s doen\u00e7as, les\u00f5es e infec\u00e7\u00f5es alteram os resultados de exames<sup>11<\/sup>.<\/p>\n<p>O grau dessas modifica\u00e7\u00f5es pode variar de acordo com a intensidade da resposta inflamat\u00f3ria, com reflexos sobre os testes sangu\u00edneos<sup>11<\/sup>. O indicador mais conhecido desse tipo de altera\u00e7\u00e3o \u00e9 a prote\u00edna C Reativa (ou somente PCR). \u00a0<strong>\u00c9 poss\u00edvel notar que como nessas circunst\u00e2ncias h\u00e1 uma redistribui\u00e7\u00e3o da maioria dos micronutrientes no organismo<sup>13<\/sup><\/strong>. Assim, an\u00e1lises funcionais (como da glutationa peroxidase) tendem a ser mais confi\u00e1veis<sup>11<\/sup>.<\/p>\n<p>Em suma, a defici\u00eancia de micronutrientes ainda \u00e9 um assunto que caminha na penumbra<sup>1<\/sup>. <strong>Enquanto sabemos da import\u00e2ncia da complementa\u00e7\u00e3o desses elementos em fun\u00e7\u00e3o das consequ\u00eancias da defici\u00eancia, em muitas situa\u00e7\u00f5es a forma adequada de identificar e manejar a condi\u00e7\u00e3o ainda \u00e9 incerta<sup>1<\/sup><\/strong>.
