Por isso, vale a pena repassar alguns pontos essenciais sobre o uso de determinados micronutrientes de acordo com as <a href=\"https:\/\/www.clinicalnutritionjournal.com\/action\/showPdf?pii=S0261-5614(22)00066-8\">principais diretrizes sobre o assunto.<\/a><\/p>\n<p>Micronutrientes englobam vitaminas e oligoelementos que, em indiv\u00edduos saud\u00e1veis, s\u00e3o supridos pela alimenta\u00e7\u00e3o, desde que ela seja diversa e balanceada<sup>1<\/sup>. No entanto, pacientes doentes e recebendo fontes alternativas de nutri\u00e7\u00e3o podem sofrer desequil\u00edbrios em alguns desses itens<strong>, seja pela falta da oferta, seja pela interfer\u00eancia em sua biodisponibilidade. <\/strong><\/p>\n<p>Al\u00e9m disso, mesmo que j\u00e1 se conhe\u00e7a a import\u00e2ncia dos micronutrientes h\u00e1 d\u00e9cadas, n\u00e3o \u00e9 raro que eles sejam subutilizados ou deixados de lado durante a terapia nutricional parenteral. Isso pode se dar, inclusive, por consequ\u00eancia da aus\u00eancia de dosagem e monitoramento adequados<sup>1<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<h2>1.\u00a0\u00a0\u00a0 Cromo<\/h2>\n<p>O<strong> cromo recebe destaque por conta de muitos estudos que avaliaram sua interfer\u00eancia em mecanismos da a\u00e7\u00e3o da insulina nos tecidos perif\u00e9ricos, melhorando a resist\u00eancia ao horm\u00f4nio<sup>2<\/sup>. <\/strong>\u00a0Sabe-se, tamb\u00e9m, que ele atua como componente de metaloenzimas (enzimas que cont\u00e9m um ou mais \u00edons met\u00e1licos na sua estrutura) e funciona como coenzima em muitos processos metab\u00f3licos (de carboidratos, prote\u00ednas, gorduras)<sup>3<\/sup>. Al\u00e9m disso, ele interfere no estado oxidativo do organismo<sup>2<\/sup>.<\/p>\n<p>No geral, a recomenda\u00e7\u00e3o de ingest\u00e3o adequada de cromo para um adulto at\u00e9 50 anos varia entre 25 \u00b5g\/dia (para mulheres) e 35 \u00b5g\/dia (para homens). Por\u00e9m, existem evid\u00eancias que tais necessidades possam ser ainda menores, tendo como refer\u00eancia a baixa absor\u00e7\u00e3o por via oral em pessoas saud\u00e1veis<sup>3<\/sup>. De todo modo, o aporte recomendado para pacientes recebendo nutri\u00e7\u00e3o parenteral costuma oscilar entre 10 e 15 \u00b5g\/dia<sup>1,4,5,6.<\/sup><\/p>\n<p><strong>Vale destacar que o n\u00edvel s\u00e9rico n\u00e3o \u00e9 a melhor forma de identificar defici\u00eancias do micronutriente: o cromo no sangue n\u00e3o reflete prontamente os estoques do elemento nos tecidos.<\/strong> \u00a0J\u00e1 a excre\u00e7\u00e3o de cromo na urina \u00e9 um bom marcador de exposi\u00e7\u00e3o \u00e0 alta quantidade do oligoelemento, mas n\u00e3o \u00e9 \u00fatil para predizer quantidade estocada nos tecidos.<\/p>\n<p>A resposta ao teste de toler\u00e2ncia oral \u00e0 glicose e a excre\u00e7\u00e3o do cromo na urina, juntos, costumam ser melhores indicadores dos seus n\u00edveis no organismo<sup>3<\/sup>. Por fim, cabe observar que os n\u00edveis de cromo s\u00e9rico podem estar alterados na presen\u00e7a de inflama\u00e7\u00e3o<sup>1,5<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<h2>2.\u00a0\u00a0\u00a0 Iodo<\/h2>\n<p>De forma ampla, o <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/qual-e-a-funcao-do-fluor-e-do-iodo-na-nutricao-parenteral\/\">iodo<\/a> desempenha papel indispens\u00e1vel no funcionamento da tireoide, tanto na regula\u00e7\u00e3o da gl\u00e2ndula, quanto na composi\u00e7\u00e3o dos horm\u00f4nios liberados por ela. <strong>Esses horm\u00f4nios, por sua vez, regulam o metabolismo do corpo, com efeitos sobre diferentes enzimas que atuam na \u201cqueima\u201d de gorduras e de carboidratos<sup>7<\/sup>.<\/strong><\/p>\n<p>Em adultos saud\u00e1veis, as necessidades di\u00e1rias de ingest\u00e3o de iodo podem variar entre 150 \u00b5g\/dia e 290 \u00b5g\/dia (a maior necessidade diz respeito \u00e0s demandas de mulheres lactantes)<sup>8<\/sup>. <strong>De todo modo, as recomenda\u00e7\u00f5es atuais estimam um aporte entre 70 \u00b5g e 150 \u00b5g para aqueles pacientes que recebem nutri\u00e7\u00e3o parenteral<sup>1<\/sup>.<\/strong><\/p>\n<p>A defici\u00eancia de iodo pode ser detectada pela redu\u00e7\u00e3o da sua excre\u00e7\u00e3o na avalia\u00e7\u00e3o da urina de 24h (em taxas abaixo de 100 \u00b5g\/24h)<sup>9<\/sup>.<\/p>\n<p><em>Veja tamb\u00e9m: <\/em><a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/importancia-oligoelementos-nutricao-parenteral\/\"><em>Entenda a import\u00e2ncia dos oligoelementos na nutri\u00e7\u00e3o parenteral <\/em><\/a><\/p>\n<p>&nbsp;<\/p>\n<h2>3.\u00a0\u00a0\u00a0 Mangan\u00eas<\/h2>\n<p>O<a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/necessidade-manganes-nutricao-parenteral\/\"> mangan\u00eas<\/a> \u00e9 um dos metais mais comuns no organismo humano<sup>9<\/sup>. <strong>Ele desempenha fun\u00e7\u00f5es essenciais na regula\u00e7\u00e3o do a\u00e7\u00facar do sangue, da energia celular, dos processos de coagula\u00e7\u00e3o e em alguns mecanismos do sistema imune, entre outros mecanismos<sup>10<\/sup><\/strong>.<\/p>\n<p>Na m\u00e9dia, a recomenda\u00e7\u00e3o de ingest\u00e3o di\u00e1ria de mangan\u00eas para adultos \u00e9 de 2 mg por dia<sup>8<\/sup>. Na nutri\u00e7\u00e3o parenteral, esse micronutriente pode aparecer como parte da composi\u00e7\u00e3o da f\u00f3rmula ou como um contaminante (em quantidades sempre muito pequenas).<\/p>\n<p>A dose ideal deve estar pr\u00f3xima a 55 \u00b5g\/dia para pacientes adultos<sup>11<\/sup>. Com isso em mente, \u00e9 preciso considerar que aportes acima de 110 \u00b5g\/dia s\u00e3o considerados excessivos<sup>11<\/sup>.<\/p>\n<p>O total de mangan\u00eas \u00e9 obtido atrav\u00e9s de an\u00e1lises do micronutriente no sangue total, no plasma, nos gl\u00f3bulos vermelhos e na urina<sup>9<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<h2>4.\u00a0\u00a0\u00a0 Ferro<\/h2>\n<p>O<a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/ferro-na-nutricao-parenteral-seguro-e-necessario\/\"> ferro,<\/a> por sua vez, \u00e9 o elemento-tra\u00e7o mais abundante no organismo humano<sup>9<\/sup>. <strong>A sua principal fun\u00e7\u00e3o no corpo \u00e9 compor o grupo de prote\u00ednas heme, respons\u00e1veis pela liga\u00e7\u00e3o, transporte e metabolismo do oxig\u00eanio. <\/strong>Uma das hemeprote\u00ednas mais conhecidas \u00e9 a hemoglobina<sup>9<\/sup>.<\/p>\n<p>Al\u00e9m disso, o ferro tamb\u00e9m pode estar dispon\u00edvel na forma n\u00e3o-heme. No entanto, \u00e9 preciso levar em conta que a defici\u00eancia desse micronutriente \u00e9 uma das mais comuns<sup>12,13<\/sup>. Na maioria dos casos, <strong>pacientes em nutri\u00e7\u00e3o parenteral devem receber entre 1 mg\/dia <\/strong>(adultos do sexo masculino e mulheres na p\u00f3s-menopausa)<strong> e 2 mg\/dia <\/strong>(mulheres no final da gesta\u00e7\u00e3o ou lactantes)<strong><sup>13<\/sup>.<\/strong><\/p>\n<p>Os m\u00e9todos mais tradicionais para mensurar a quantidade de ferro no organismo envolvem a contagem de hemoglobina, ferritina (forma de armazenamento de ferro), satura\u00e7\u00e3o de transferrina e capacidade total de liga\u00e7\u00e3o do ferro<sup>22<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<h2>5.\u00a0\u00a0\u00a0 Zinco<\/h2>\n<p>O <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/zinco-importancia-em-nutricao-parenteral\/\">zinco <\/a>aparece no organismo humano sob a forma de mais de 300 metaloenzimas. Elas desempenham fun\u00e7\u00f5es essenciais em praticamente todos os processos metab\u00f3licos do corpo. <strong>A partir disso, as fun\u00e7\u00f5es desse micronutriente no organismo podem ser agrupadas em tr\u00eas classes<sup>9<\/sup>: <\/strong><\/p>\n<ul>\n<li><strong>Estruturais <\/strong>(participando da composi\u00e7\u00e3o de prote\u00ednas);<\/li>\n<li><strong>Catal\u00edticas <\/strong>(participante de enzimas como as oxidorredutases, transferases, hidrolases, entre outras);<\/li>\n<li><strong>Regulat\u00f3rias <\/strong>(atuando como sinalizador nos sistemas end\u00f3crino, par\u00e1crino e aut\u00f3crino).<\/li>\n<\/ul>\n<p>As recomenda\u00e7\u00f5es di\u00e1rias de zinco variam bastante. Resumidamente, \u00e9 poss\u00edvel considerar patamares entre 8mg e 15 mg em adultos<sup>9<\/sup>. Em pacientes recebendo nutri\u00e7\u00e3o parenteral, as necessidades adicionais de zinco costumam ser pequenas, desde que n\u00e3o haja perdas<sup>9<\/sup>. <strong>Assim sendo, 3 a 12 mg\/dia costumam ser suficientes para a maioria dos pacientes, incluindo aqueles com problemas gastrointestinais<sup>14<\/sup><\/strong>.<\/p>\n<p>A presen\u00e7a de zinco no organismo pode ser avaliada atrav\u00e9s do sangue, do plasma, da urina ou mesmo dos cabelos<sup>9<\/sup>.
