Veja qual o papel desse amino\u00e1cido na nutri\u00e7\u00e3o parenteral"},"content":{"rendered":"<p>Entender o que \u00e9 a glutamina envolve compreender como ela se diferencia, em alguns aspectos, da maioria dos amino\u00e1cidos presentes no organismo humano. <strong>Na pr\u00e1tica, ela \u00e9 classificada como um amino\u00e1cido condicionalmente essencial<sup>1<\/sup>. <\/strong><\/p>\n<p>Ou seja, em condi\u00e7\u00f5es normais,<strong> o organismo consegue produzi-la em quantidade suficiente, mas isso pode n\u00e3o ser o bastante em situa\u00e7\u00f5es prolongadas de estresse<sup>2,3<\/sup><\/strong>. Exemplos disso s\u00e3o epis\u00f3dios de inflama\u00e7\u00e3o, infec\u00e7\u00e3o, p\u00f3s-operat\u00f3rio ou outros quadros catab\u00f3licos, incluindo a desnutri\u00e7\u00e3o<sup>1<\/sup>.<\/p>\n<p>Nesse contexto, parece haver benef\u00edcios na indica\u00e7\u00e3o da glutamina em pacientes recebendo nutri\u00e7\u00e3o parenteral em determinadas circunst\u00e2ncias. Isso \u00e9 o que vamos explorar neste artigo.<\/p>\n<p>&nbsp;<\/p>\n<h2>Afinal, o que \u00e9 a glutamina e qual seu papel no organismo?<\/h2>\n<p>Para come\u00e7ar, vale relembrar o papel dos amino\u00e1cidos em geral: eles s\u00e3o \u201cblocos\u201d que permitem a constru\u00e7\u00e3o das c\u00e9lulas e tecidos<sup>4<\/sup>. <strong>Eles podem ser considerados essenciais (que o organismo n\u00e3o \u00e9 capaz de sintetizar e devem ser fornecidos pela dieta) ou n\u00e3o essenciais (que s\u00e3o produzidos endogenamente pelo corpo)<sup>1<\/sup>.<\/strong><\/p>\n<p>Nesse cen\u00e1rio, a glutamina se destaca como um dos amino\u00e1cidos mais vers\u00e1teis no organismo humano (como j\u00e1 destacado na introdu\u00e7\u00e3o, inclusive). <strong>Em praticamente toda c\u00e9lula ela est\u00e1 envolvida em processos que v\u00e3o desde a s\u00edntese de nucleot\u00eddeos e antioxidantes at\u00e9 a manuten\u00e7\u00e3o de uma s\u00e9rie de fun\u00e7\u00f5es essenciais \u00e0 preserva\u00e7\u00e3o da estrutura e da funcionalidade celular<sup>5<\/sup><\/strong><sup>,6,7<\/sup>.<\/p>\n<p>Entretanto, a partir do momento em que o organismo se encontra sob estresse, a quantidade de glutamina dispon\u00edvel pode cair consideravelmente. Tal situa\u00e7\u00e3o \u00e9 relativamente comum e se reflete sobretudo nos m\u00fasculos, no sangue e nas c\u00e9lulas imunol\u00f3gicas<sup>8,9,10<\/sup>.<\/p>\n<p>Desse modo, o paciente pode apresentar redu\u00e7\u00e3o da fun\u00e7\u00e3o imunol\u00f3gica, diminui\u00e7\u00e3o da energia celular dispon\u00edvel e uma s\u00e9rie de danos org\u00e2nicos<sup>11<\/sup>.<\/p>\n<p>&nbsp;<\/p>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-full wp-image-1970\" src=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/06\/lam158-imagem.png\" alt=\"\" width=\"941\" height=\"600\" srcset=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/06\/lam158-imagem.png 941w, https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/06\/lam158-imagem-300x191.png 300w, https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/06\/lam158-imagem-768x490.png 768w, https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/wp-content\/uploads\/sites\/3\/2024\/06\/lam158-imagem-580x370.png 580w\" sizes=\"auto, (max-width: 941px) 100vw, 941px\" \/><\/p>\n<p>&nbsp;<\/p>\n<h2>Quais os benef\u00edcios da adi\u00e7\u00e3o desse amino\u00e1cido na nutri\u00e7\u00e3o parenteral?<\/h2>\n<p>Dentro das evid\u00eancias dispon\u00edveis, parece haver um efeito positivo da introdu\u00e7\u00e3o da glutamina na nutri\u00e7\u00e3o parenteral de pacientes em determinados contextos<sup>12<\/sup>.<\/p>\n<p>Uma<a href=\"https:\/\/clinicalnutritionespen.com\/article\/S2405-4577(16)30279-0\/fulltext\"> revis\u00e3o sistem\u00e1tica publicada em 2017<\/a>, por exemplo, reuniu 15 ensaios cl\u00ednicos randomizados para avaliar o uso da glutamina em pacientes cr\u00edticos. Ao todo, foi poss\u00edvel analisar os dados de 842 indiv\u00edduos, em sua maioria com quadros de <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/nutricao-e-cicatrizacao-de-feridas-e-lesoes-quais-as-recomendacoes\/\">trauma<\/a>, infec\u00e7\u00e3o ou cirurgias extensas <sup>13<\/sup>.<\/p>\n<p>No fim, foi poss\u00edvel concluir que a inclus\u00e3o da glutamina em uma popula\u00e7\u00e3o bem selecionada e sem contraindica\u00e7\u00f5es a seu uso promoveu<sup>13<\/sup>:<\/p>\n<ul>\n<li>Redu\u00e7\u00e3o de complica\u00e7\u00f5es decorrentes de infec\u00e7\u00f5es;<\/li>\n<li>Redu\u00e7\u00e3o do tempo de perman\u00eancia em unidade de terapia intensiva;<\/li>\n<li>Redu\u00e7\u00e3o da dura\u00e7\u00e3o do per\u00edodo de ventila\u00e7\u00e3o mec\u00e2nica;<\/li>\n<li>Redu\u00e7\u00e3o da mortalidade hospitalar.<\/li>\n<\/ul>\n<p>Ainda de acordo com o estudo em quest\u00e3o, o uso da glutamina na <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/indicacao-nutricao-parenteral\/\">nutri\u00e7\u00e3o parenteral<\/a> pode oferecer ganhos em duas frentes. <strong>De um lado, tal interven\u00e7\u00e3o parece relevante para alcan\u00e7ar melhores desfechos cl\u00ednicos em pacientes cr\u00edticos cir\u00fargicos ou v\u00edtima de traumas. De outro, a introdu\u00e7\u00e3o desse amino\u00e1cido na f\u00f3rmula administrada tamb\u00e9m confere benef\u00edcios econ\u00f4micos no manejo dos recursos hospitalares<sup>13<\/sup><\/strong>.<\/p>\n<p>Mas o que \u00e9 importante levar em conta nessa equa\u00e7\u00e3o? A revis\u00e3o apresenta resultados de destaque no contexto cl\u00ednico e econ\u00f4mico, <strong>mas \u00e9 muito importante que a glutamina, na forma de dipept\u00eddeo, seja ofertada juntamente com o suporte da nutri\u00e7\u00e3o parenteral capaz de atender \u00e0s necessidades cal\u00f3ricas e proteicas do paciente.<\/strong><\/p>\n<p>Com isso, a glutamina administrada serve como precursora em v\u00e1rias vias biossint\u00e9ticas em vez de apenas atuar como combust\u00edvel energ\u00e9tico<sup>13<\/sup>. A mais conhecida dessas combina\u00e7\u00f5es \u00e9 o L-alanil-L-glutamina (ou s\u00f3 Ala-Gln)<sup>14<\/sup>.<\/strong><\/p>\n<p>A glutamina pode ser inclu\u00edda na administra\u00e7\u00e3o da nutri\u00e7\u00e3o parenteral sempre de acordo com as condi\u00e7\u00f5es dos pacientes, conforme descrito nas principais diretrizes sobre o tema. Assim sendo, esse amino\u00e1cido pode ser importante para indiv\u00edduos com pancreatite aguda grave, por exemplo, mas o n\u00edvel de evid\u00eancia para essa recomenda\u00e7\u00e3o ainda \u00e9 baixo<sup>15<\/sup>.<\/p>\n<p>Adicionalmente, a suplementa\u00e7\u00e3o tende a oferecer benef\u00edcios consider\u00e1veis em pacientes imunossuprimidos, em que o catabolismo\/hipercatabolismo afeta a homeostase do metabolismo dos amino\u00e1cidos<sup>5<\/sup>.<\/p>\n<p><strong>Seja como for, a <\/strong><a href=\"https:\/\/www.clinicalnutritionjournal.com\/article\/S0261-5614(18)32432-4\/fulltext\"><strong>European Society for Clinical Nutrition and Metabolism,<\/strong><\/a><strong> (Espen) orienta que somente pacientes cr\u00edticos est\u00e1veis poderiam ser avaliados quanto ao uso da glutamina na nutri\u00e7\u00e3o parenteral, como na revis\u00e3o apresentada anteriormente. <\/strong><\/p>\n<p><strong>\u00a0<\/strong>Vale ainda destacar que n\u00e3o existem dados sobre a administra\u00e7\u00e3o de glutamina parenteral a longo prazo. Por isso, a maioria dos ensaios envolve o seu uso adicionada \u00e0 nutri\u00e7\u00e3o parenteral, mas n\u00e3o por mais de 14 dias<sup>16<\/sup>.<\/p>\n<p>Da mesma maneira, <strong>a <\/strong><a href=\"http:\/\/www.braspen.periodikos.com.br\/article\/10.37111\/braspenj.diretrizDOENTEGRAVE\/pdf\/braspen-38-2%2C+Supl+2-6537d6b0a953950ad57860b3.pdf\"><strong>Sociedade Brasileira de Nutri\u00e7\u00e3o Parenteral e Enteral <\/strong><\/a><strong>(Braspen) contraindica o uso de glutamina em pacientes na fase aguda de doen\u00e7a grave, com disfun\u00e7\u00e3o org\u00e2nica m\u00faltipla, disfun\u00e7\u00e3o renal, disfun\u00e7\u00e3o hep\u00e1tica ou instabilidade hemodin\u00e2mica e em doses acima de 0,5 g\/kg\/d<sup>15<\/sup>. <\/strong><\/p>\n<p>Por fim, com o foco em pacientes cir\u00fargicos, o <em>guideline<\/em> da <a href=\"https:\/\/www.espen.org\/files\/ESPEN-Guidelines\/ESPEN_practical_guideline_Clinical_nutrition_in_surgery.pdf\">Espen (European Society for Clinical Nutrition and Metabolism)<\/a> <strong>orienta que a suplementa\u00e7\u00e3o de glutamina deve ser avaliada em pacientes que n\u00e3o podem ser nutridos adequadamente por via enteral <\/strong>e, portanto, passam a necessitar de nutri\u00e7\u00e3o parenteral exclusiva<sup>17<\/sup>.<\/p>\n<p>Agora que voc\u00ea sabe o que \u00e9 a glutamina e qual seu papel na nutri\u00e7\u00e3o parenteral, <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/diferentes-aminoacidos-nutricao-parenteral\/\">confira algumas das vantagens do uso adequado de diferentes amino\u00e1cidos nas f\u00f3rmulas administradas.<\/a><\/p>\n<p>&nbsp;<\/p>\n<p>&nbsp;<\/p>\n<p>Refer\u00eancias:<\/p>\n<ol>\n<li>Fresenius Kabi.
