{"id":1794,"date":"2022-06-15T15:08:16","date_gmt":"2022-06-15T18:08:16","guid":{"rendered":"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/?p=1794"},"modified":"2023-10-24T12:50:44","modified_gmt":"2023-10-24T15:50:44","slug":"oleo-de-peixe-e-seu-papel-nutricional-no-paciente-critico","status":"publish","type":"post","link":"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/oleo-de-peixe-e-seu-papel-nutricional-no-paciente-critico\/","title":{"rendered":"\u00d3leo de peixe e seu papel nutricional no paciente cr\u00edtico"},"content":{"rendered":"<p>O \u00f3leo de peixe (FO) \u00e9 reconhecido por sua riqueza em <a href=\"https:\/\/unidospelanutricaoclinica.com.br\/w3-nutricao-parenteral\/\"><strong>\u00f4mega 3 (<\/strong><strong>\u03c93)<\/strong>,<\/a> <strong>composto lip\u00eddico formado pelo \u00e1cido alfa-linol\u00eanico (ALA), \u00e1cido eicosapentaenoico (EPA) e \u00e1cido docosa-hexaenoico (DHA).<\/strong> Todos eles fazem parte do grupo de \u00e1cidos graxos poli-insaturados, que passou a ser mais abundante em emuls\u00f5es lip\u00eddicas (EL) a partir dos anos 2000, quando foi incorporado a elas o \u00f3leo de peixe (FO).<\/p>\n<p>Em raz\u00e3o da riqueza de EPA e DHA no FO, que t\u00eam potencial imunomodulat\u00f3rio e efeito ben\u00e9fico em processos inflamat\u00f3rios e desenvolvimento de resposta imune, pesquisadores passaram a avaliar os benef\u00edcios que a inclus\u00e3o do \u00f4mega 3, via introdu\u00e7\u00e3o de \u00f3leo de peixe na dieta, poderiam gerar aos pacientes em estado cr\u00edtico.<\/p>\n<p>\u00c9 o que voc\u00ea ver\u00e1, a seguir, neste artigo. Basta continuar a leitura!<\/p>\n<h2>Recomenda\u00e7\u00f5es de lip\u00eddios em pacientes cr\u00edticos<\/h2>\n<p>A <a href=\"https:\/\/www.espen.org\/files\/ESPEN-Guidelines\/ESPEN_guideline-on-clinical-nutrition-in-the-intensive-care-unit.pdf\">European Society for Clinical Nutrional an Metabolism (ESPEN)<\/a> aponta um estudo em que <strong>pacientes pedi\u00e1tricos que receberam emuls\u00e3o lip\u00eddica (EL) de \u00f3leo de peixe puro n\u00e3o desenvolveram, ap\u00f3s meses, defici\u00eancia de \u00e1cidos graxos essenciais (estudo 192 da ESPEN))<\/strong>. De acordo com a ESPEN, a administra\u00e7\u00e3o pode ocorrer via enteral ou parenteral, associada a carboidratos.<a class=\"bibliography-sup\" title=\"1.\tSINGER, P.; et al. ESPEN Guideline on clinical Nutrition in the Intensive Care Unit: 2018; P. 62; Dispon\u00edvel em https:\/\/www.espen.org\/files\/ESPEN-Guidelines\/ESPEN_guideline-on-clinical-nutrition-in-the-intensive-care-unit.pdf\" href=\"#\" data-bibliography-link=\"\">\u00b9<\/a>.<\/p>\n<p>Contudo, ainda segundo a ESPEN, a absor\u00e7\u00e3o de lip\u00eddios \u00e9 prejudicada em pacientes com doen\u00e7as cr\u00edticas, uma vez que o metabolismo fica modificado nessas situa\u00e7\u00f5es. A entidade alerta, portanto, que a administra\u00e7\u00e3o de quantidades elevadas de carboidratos e de lip\u00eddios pode levar \u00e0 hiperglicemia e causar anormalidades no f\u00edgado. Sendo assim, a recomenda\u00e7\u00e3o \u00e9 de monitoramento rigoroso de triglicer\u00eddeos e testes de fun\u00e7\u00e3o hep\u00e1tica para orientar o m\u00e9dico no estabelecimento da melhor propor\u00e7\u00e3o carboidrato\/gordura.<a class=\"bibliography-sup\" title=\"1.\tSINGER, P.; et al. ESPEN Guideline on clinical Nutrition in the Intensive Care Unit: 2018; P. 62; Dispon\u00edvel em https:\/\/www.espen.org\/files\/ESPEN-Guidelines\/ESPEN_guideline-on-clinical-nutrition-in-the-intensive-care-unit.pdf\" href=\"#\" data-bibliography-link=\"\">\u00b9<\/a><\/p>\n<h2>Estudos sugerem benef\u00edcios cl\u00ednicos do uso de \u00f3leo de peixe em pacientes cr\u00edticos<\/h2>\n<p>Com objetivo de avaliar evid\u00eancias de estudos laboratoriais e com animais que demonstravam benef\u00edcios cl\u00ednicos do uso de EL intravenosa contendo FO, pesquisadores de universidades na Turquia, Nova Zel\u00e2ndia, Uruguai e Estados Unidos realizaram revis\u00e3o para observar os efeitos ben\u00e9ficos sobre inflama\u00e7\u00f5es e resposta imune entre aqueles que receberam \u00f3leo de peixe.<\/p>\n<p>Para tal, foram avaliados 34 estudos cl\u00ednicos, com considera\u00e7\u00f5es sobre n\u00edveis de marcadores inflamat\u00f3rios e resposta imunol\u00f3gica; taxas de infec\u00e7\u00e3o e sepse; e, ainda, resultados cl\u00ednicos gerais<a class=\"bibliography-sup\" title=\"2.\tABBASOGLU, O.; et al. Dispon\u00edvel em: https:\/\/aspenjournals.onlinelibrary.wiley.com\/doi\/10.1177\/0148607117721907\" href=\"#\" data-bibliography-link=\"\">2<\/a>.<\/p>\n<p>Os <strong>resultados mostraram algumas evid\u00eancias de que EL contendo \u00f3leo de peixe foi capaz de reduzir a taxa de fibrila\u00e7\u00e3o atrial p\u00f3s-operat\u00f3ria<a class=\"bibliography-sup\" title=\"2.\tABBASOGLU, O.; et al. No entanto, ao observar n\u00edveis de marcadores inflamat\u00f3rios e imunol\u00f3gicos, as evid\u00eancias foram conflitantes. <strong>Os estudos tamb\u00e9m relataram poucas diferen\u00e7as entre resultados cl\u00ednicos e taxas de infec\u00e7\u00e3o e sepse comparando o uso de \u00f3leo de peixe com outras emuls\u00f5es intravenosas<\/strong>. Veja os achados da meta-an\u00e1lise na figura!<\/p>\n<p>&nbsp;<\/p>\n<h3><\/h3>\n<h3>EPA, DHA e risco de doen\u00e7a arterial coronariana<\/h3>\n<p>Corroborando os ind\u00edcios de que o uso de \u00f4mega 3 poderia beneficiar pacientes com risco card\u00edaco, a <a href=\"https:\/\/www.portal.cardiol.br\/\">Sociedade Brasileira de Cardiologia<\/a> em seu documento de posicionamento sobre o consumo de gorduras e sa\u00fade cardiovascular apresenta meta-an\u00e1lise recente de estudos prospectivos de coorte para verificar a associa\u00e7\u00e3o entre o consumo de EPA, DHA e o risco de doen\u00e7a arterial coronariana (DAC). Os resultados demonstraram benef\u00edcios significativos principalmente na popula\u00e7\u00e3o com alto risco card\u00edaco, com redu\u00e7\u00e3o do risco coron\u00e1rio de acordo com eleva\u00e7\u00e3o do consumo de EPA+DHA. \u201cEm um ensaio randomizado com paciente com DAC, a suplementa\u00e7\u00e3o com aproximadamente 1,5 g\/dia de \u00e1cido graxo \u03c93 por dois anos provocou menos progress\u00e3o e mais regress\u00e3o da aterosclerose coronariana, medida por angiografia invasiva quantitativa, em rela\u00e7\u00e3o ao uso de placebo, embora as diferen\u00e7as tenham sido pequenas. <a class=\"bibliography-sup\" title=\"4.\tIZAR, M.C.O. et al. Dispon\u00edvel em: https:\/\/abccardiol.org\/article\/posicionamento-sobre-o-consumo-de-gorduras-e-saude-cardiovascular-2021\/\" href=\"#\" data-bibliography-link=\"\">4<\/a><\/p>\n<p>&nbsp;<\/p>\n<h3>O que diz quem recomenda o uso de \u00f4mega 3 em pacientes cr\u00edticos!<\/h3>\n<p>Pesquisadores do <a href=\"https:\/\/www.ohsu.edu\/\">Departamento de Cirurgia e Gastroenterologia da Universidade de Sa\u00fade e Ci\u00eancia de Oregon<\/a>, nos Estados Unidos, por outro lado, afirmam que a f\u00f3rmula ideal para o paciente em ambiente de terapia intensiva deve ir al\u00e9m do que \u00e9 considerado tradicionalmente (f\u00f3rmula padr\u00e3o polim\u00e9rica, via enteral, para os que toleram, de 1,0-1,5 kcal\/mL).
