Nos \u00faltimos tempos, tal componente vem se tornando um item essencial nas f\u00f3rmulas mais modernas, em contraposi\u00e7\u00e3o \u00e0quelas desenvolvidas com \u00f3leo de soja, TCM e\/ou \u00f3leo de oliva (2).<\/p>\n<p>Mas afinal, o que \u00e9 o \u00f4mega-3, quais s\u00e3o suas fun\u00e7\u00f5es e qual \u00e9 o seu papel da nutri\u00e7\u00e3o parenteral? \u00c9 o que iremos descobrir hoje.<\/p>\n<h2>O que \u00e9 \u00f4mega-3?<\/h2>\n<p>O \u00f4mega 3 (\u03c9-3) \u00e9 um grupo de \u00e1cidos graxos poli-insaturados de cadeia longa, fonte de energia e essenciais para nosso organismo.<\/p>\n<p>Os principais tipos de \u00f4mega 3 s\u00e3o o <strong>\u00e1cido alfa-linol\u00eanico (ALA)<\/strong>, o <strong>\u00e1cido eicosapentaenoico (EPA)<\/strong> e o <strong>\u00e1cido docosahexaenoico (DHA)<\/strong> (3).<\/p>\n<p>O ALA \u00e9 encontrado principalmente em \u00f3leos vegetais, como os de linha\u00e7a, soja e canola. J\u00e1 o DHA e o EPA s\u00e3o encontrados em peixes e outros frutos do mar (3).<\/p>\n<p>Na nutri\u00e7\u00e3o parenteral, o <strong>\u00f3leo de peixe<\/strong> \u00e9 a fonte de \u00f4mega-3 para os pacientes hospitalizados, composto por uma longa cadeia condicionalmente essencial de \u00e1cido eicosapentaenoico\u00a0 (EPA, C20: 5 \u03c9-3) e \u00e1cido docosahexaenoico (DHA, C22:6 \u03c9-3) (2).<\/p>\n<h2>Qual o papel nutricional do \u00f4mega 3?<\/h2>\n<p>Os \u00e1cidos graxos da fam\u00edlia \u00f4mega 3 s\u00e3o componentes importantes das membranas celulares. Al\u00e9m de <strong>fornecer energia<\/strong>, eles possuem diversas fun\u00e7\u00f5es no <strong>cora\u00e7\u00e3o, vasos sangu\u00edneos, pulm\u00f5es, sistema imunol\u00f3gico, sistema nervoso e sistema end\u00f3crino<\/strong> (3).<\/p>\n<p>Seu papel nutricional, contudo, pode variar de acordo com a faixa et\u00e1ria e condi\u00e7\u00e3o de sa\u00fade de cada indiv\u00edduo. Por exemplo, o suprimento adequado de PUFAs \u03c9-3, especialmente DHA, tem papel fundamental no <strong>desenvolvimento visual, neural e comportamental<\/strong> <strong>em beb\u00eas<\/strong> (4).<\/p>\n<p>J\u00e1 EPA e DHA <strong>reduzem a produ\u00e7\u00e3o de citocinas e subst\u00e2ncias pr\u00f3-inflamat\u00f3rias<\/strong> e, assim, favorecem a resposta imune normal. Isso pode ser ben\u00e9fico em pacientes com comprometimento do sistema imunol\u00f3gico, como na sepse (5, 6).<\/p>\n<p>De modo geral, o consumo adequado de \u00f4mega 3 j\u00e1 demonstrou diversos benef\u00edcios para a sa\u00fade de acordo com a literatura cient\u00edfica. Estes incluem (3):<\/p>\n<ul>\n<li>Preven\u00e7\u00e3o e tratamento de doen\u00e7as cardiovasculares<\/li>\n<li>Desenvolvimento infantil (exp: maior peso ao nascer)<\/li>\n<li>Preven\u00e7\u00e3o do c\u00e2ncer<\/li>\n<li>Menor risco de deteriora\u00e7\u00e3o cognitiva<\/li>\n<li>Menor risco de degenera\u00e7\u00e3o macular relacionada \u00e0 idade (DMRI)<\/li>\n<li>Controle de artrite reumatoide<\/li>\n<li>Entre outros<\/li>\n<\/ul>\n<h2>Benef\u00edcios do \u00f4mega-3 na nutri\u00e7\u00e3o parenteral<\/h2>\n<p>H\u00e1 um volume suficiente de evid\u00eancias apontando para os benef\u00edcios do \u00f4mega 3 nas emuls\u00f5es lip\u00eddicas que comp\u00f5em a nutri\u00e7\u00e3o parenteral.<\/p>\n<p>Uma revis\u00e3o de 49 artigos de ensaios cl\u00ednicos controlados e randomizados mostrou que f\u00f3rmulas de nutri\u00e7\u00e3o parenteral enriquecidas com \u00f4mega-3 <strong>reduzem o risco de infec\u00e7\u00e3o (em 40%) e sepse (em 56%)<\/strong> quando comparadas \u00e0s alternativas sem tal componente. Al\u00e9m disso, esses pacientes permaneciam at\u00e9 dois dias a menos em ambiente de terapia intensiva (7).<\/p>\n<p>Outra revis\u00e3o sistem\u00e1tica mais recente comparou resultados em quatro grupos de pacientes, conforme a composi\u00e7\u00e3o da emuls\u00e3o lip\u00eddica (2):<\/p>\n<ol>\n<li>Com \u00f3leo de peixe (como parte predominante da emuls\u00e3o, mas poderiam conter outros \u00f3leos como soja, TCM e oliva)<\/li>\n<li>Com \u00f3leo de oliva e soja<\/li>\n<li>Com TCM e \u00f3leo de soja<\/li>\n<li>Com \u00f3leo de soja puro<\/li>\n<\/ol>\n<p>Os autores descobriram que as ELs com \u00f3leo de peixe (dose m\u00e9dia di\u00e1ria entre 0,1 e 0,2 g\/kg) representaram a interven\u00e7\u00e3o mais eficaz na melhoria dos resultados cl\u00ednicos em pacientes adultos hospitalizados, dentre eles (2):<\/p>\n<ul>\n<li><strong>Menor risco de infec\u00e7\u00e3o <\/strong><\/li>\n<li><strong>Menor risco de sepse<\/strong><\/li>\n<li><strong>Tempo de interna\u00e7\u00e3o consideravelmente reduzido<\/strong><\/li>\n<li><strong>Mortalidade hospitalar reduzida<\/strong><\/li>\n<\/ul>\n<p>Segundo os pesquisadores, os resultados mostram que EL\u2019s com \u00f3leo de peixe devem ser o padr\u00e3o de tratamento, particularmente em <strong>situa\u00e7\u00f5es associadas a um estado hipermetab\u00f3lico e\/ou hiperinflamat\u00f3rio<\/strong>, por exemplo, em doen\u00e7a cr\u00edtica ou p\u00f3s-cirurgia (2).<\/p>\n<h2>O que dizem as diretrizes?<\/h2>\n<p>Dado ao extenso corpo de evid\u00eancias cient\u00edficas, diversas sociedades de sa\u00fade recomendam o uso de \u00f4mega 3 na nutri\u00e7\u00e3o parenteral.<\/p>\n<p>Estas incluem a Sociedade Europeia de Nutri\u00e7\u00e3o Cl\u00ednica e Metabolismo (ESPEN) (8), a Organiza\u00e7\u00e3o Canadense de Nutri\u00e7\u00e3o em Cuidados Cr\u00edticos (9) e Sociedade Brasileira de Nutri\u00e7\u00e3o Parenteral e Enteral (BRASPEN) (10).<\/p>\n<p>Em geral, a dose recomendada \u00e9 de <strong>0,1 a 0,2 g\/kg\/dia <\/strong>de EPA e DHA (8, 10).<\/p>\n<p>Segundos os especialistas da BRASPEN, a composi\u00e7\u00e3o da EL deve depender do perfil metab\u00f3lico de cada indiv\u00edduo:<\/p>\n<ul>\n<li><strong>Pacientes hipermetab\u00f3licos<\/strong>: ELs contendo triglicer\u00eddeos de cadeia m\u00e9dia (TCM), \u00f3leo de oliva ou \u00f3leo de peixe.<\/li>\n<li><strong>Pacientes em estado cr\u00edtico<\/strong>: ELs contendo apenas \u00f3leo de peixe, de modo a promover a melhor modula\u00e7\u00e3o da resposta imunol\u00f3gica.<\/li>\n<\/ul>\n<p>Os autores tamb\u00e9m recomendam nutri\u00e7\u00e3o parenteral com \u00f4mega 3 para grupos de pacientes espec\u00edficos, como c\u00e2ncer, cuidados perioperat\u00f3rios, sepse, doen\u00e7a grave, entre outros (10).<\/p>\n<h2>O que podemos concluir?<\/h2>\n<p>Em suma, o uso de \u00f4mega 3 na nutri\u00e7\u00e3o parenteral tem sido amplamente estudado e recomendado devido aos benef\u00edcios cl\u00ednicos observados, incluindo menor risco de infec\u00e7\u00f5es, redu\u00e7\u00e3o do tempo de interna\u00e7\u00e3o e melhor modula\u00e7\u00e3o da resposta imune.<\/p>\n<p>Com base nas evid\u00eancias cient\u00edficas, diretrizes nacionais e internacionais refor\u00e7am a inclus\u00e3o de emuls\u00f5es lip\u00eddicas enriquecidas com \u00f4mega-3 como parte do suporte nutricional de pacientes hospitalizados, especialmente em condi\u00e7\u00f5es cr\u00edticas e inflamat\u00f3rias.<\/p>\n<p>De todo modo, qualquer abordagem deve ser proposta de forma personalizada, com o acompanhamento de uma equipe especializada capaz de identificar as necessidades e monitorar os resultados obtidos.<\/p>\n<p>&nbsp;<\/p>\n<p><strong>Refer\u00eancias:<\/strong><\/p>\n<p>1 &#8211; Calder PC, Adolph M, Deutz NE, et al.
