Ele exerce in\u00fameras fun\u00e7\u00f5es fisiol\u00f3gicas, e \u00e9 particularmente necess\u00e1rio para o funcionamento do sistema imunol\u00f3gico e para o crescimento.<sup>1<\/sup><\/p>\n<p>Por ser t\u00e3o importante, observar a ingest\u00e3o adequada desse oligoelemento tamb\u00e9m \u00e9 necess\u00e1rio nas pessoas que recebem nutri\u00e7\u00e3o parenteral. Esses pacientes est\u00e3o mais suscetiveis \u00e0 defici\u00eancia do mineral e a serem afetados pelas suas consequ\u00eancias cl\u00ednicas. Assim, as diretrizes nutricionais recomendam o fornecimento de zinco desde o in\u00edcio da nutri\u00e7\u00e3o parenteral.<sup>1<\/sup><\/p>\n<p>Neste artigo, vamos abordar a import\u00e2ncia do zinco para os pacientes que recebem nutri\u00e7\u00e3o parenteral. Confira a seguir.<\/p>\n<hr \/>\n<p><strong>Leia tamb\u00e9m: <\/strong><a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/ferro-na-nutricao-parenteral-seguro-e-necessario\/\">Ferro na nutri\u00e7\u00e3o parenteral: por que \u00e9 seguro e necess\u00e1rio?<\/a><\/p>\n<hr \/>\n<h2>Papel do zinco no organismo<\/h2>\n<p>O zinco participa de diversos processos, que podem ser divididos em catal\u00edticos (ou seja, acelaram rea\u00e7\u00f5es qu\u00edmicas no organismo), estruturais (s\u00e3o parte formadora da disposi\u00e7\u00e3o de prote\u00ednas e enzimas) e regulat\u00f3rios (atuando, por exemplo, na a\u00e7\u00e3o de horm\u00f4nios, s\u00edntese proteica e divis\u00e3o celular)<sup>1<\/sup>.<\/p>\n<h2>Zinco na nutri\u00e7\u00e3o parenteral<\/h2>\n<p>A oferta de zinco ao paciente que recebe nutri\u00e7\u00e3o parenteral \u00e9 amplamente recomendada para prevenir quadros de defici\u00eancia que poderiam prejudicar o estado geral de sa\u00fade, bem como para garantir que tenham a disponibilidade necess\u00e1ria do nutriente para o funcionamento ideal do metabolismo.<sup>1<\/sup><\/p>\n<p>O zinco \u00e9 fundamental para que esses pacientes, por exemplo, possam desempenhar adequadamente processo essenciais como cicatriza\u00e7\u00e3o de feridas, restaura\u00e7\u00e3o do balan\u00e7o positivo de nitrog\u00eanio no p\u00f3s-operat\u00f3rio e defesa imunol\u00f3gica contra sepse<sup>1<\/sup>.<\/p>\n<p>Tamb\u00e9m \u00e9 importante entender que essas pessoas t\u00eam maior risco de defici\u00eancia por estarem expostas a um conjunto de fatores, que podem se apresentar simultaneamente, como no quadro a seguir.<sup>1<\/sup><\/p>\n<h3>Fatores que predisp\u00f5em \u00e0 defici\u00eancia de zinco em pacientes que recebem nutri\u00e7\u00e3o parenteral<sup>1<\/sup><\/h3>\n<table>\n<tbody>\n<tr>\n<td><strong>Fator<\/strong><\/td>\n<td><strong>Causas subjacentes<\/strong><\/td>\n<\/tr>\n<tr>\n<td>Defici\u00eancia preexistente<\/td>\n<td>Baixa ingest\u00e3o de zinco, perda de apetite, aumento da demanda para crescimento, gravidez ou lacta\u00e7\u00e3o.<\/td>\n<\/tr>\n<tr>\n<td>Absor\u00e7\u00e3o diminu\u00edda<\/td>\n<td>\u00c1rea de superf\u00edcie de absor\u00e7\u00e3o diminu\u00edda, inibidores diet\u00e9ticos (\u00e1cido f\u00edtico, ferro), m\u00e1 absor\u00e7\u00e3o, doen\u00e7a inflamat\u00f3ria intestinal.<\/td>\n<\/tr>\n<tr>\n<td>Perdas do trato gastrointestinal<\/td>\n<td>Diarreia, f\u00edstulas<\/td>\n<\/tr>\n<tr>\n<td>Perda urin\u00e1ria<\/td>\n<td>Infus\u00e3o de ciste\u00edna, sepse, medicamentos.<\/td>\n<\/tr>\n<tr>\n<td>Oferta diminu\u00edda<\/td>\n<td>Forma\u00e7\u00e3o de complexo em solu\u00e7\u00e3o de nutri\u00e7\u00e3o parenteral, ader\u00eancia \u00e0s linhas do sistema de infus\u00e3o.<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<hr \/>\n<p><strong>Leia tamb\u00e9m:<\/strong> <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/o-que-e-desnutricao-relacionada-doenca\/\">O que \u00e9 desnutri\u00e7\u00e3o relacionada \u00e0 doen\u00e7a?<\/a><\/p>\n<hr \/>\n<h3>Pediatria<\/h3>\n<p>O artigo <a href=\"https:\/\/nutricion.sochipe.cl\/subidos\/catalogo3\/zinc.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">\u201cZinc: Physiology, Deficiency, and Parenteral Nutrition\u201d<\/a> explica que beb\u00eas prematuros tamb\u00e9m correm risco de defici\u00eancia porque \u00e9 no \u00faltimo trimestre da gravidez que a maior parte do zinco \u00e9 transferida da m\u00e3e para o feto. Da mesma forma, rec\u00e9m-nascidos submetidos a cirurgias s\u00e3o mais suscet\u00edveis devido ao estresse operat\u00f3rio, que aumenta as perdas urin\u00e1rias de zinco.<sup>2<\/sup> Al\u00e9m disso, a defici\u00eancia de zinco ocorre em 67% das crian\u00e7as com insufici\u00eancia intestinal.<sup>3 <\/sup>A defici\u00eancia em neonatos pode comprometer seu crescimento e funcionamento do sistema imunol\u00f3gico.<sup>1<\/sup><\/p>\n<h2>Suplementa\u00e7\u00e3o<\/h2>\n<p>A melhor forma de ter acesso ao zinco \u00e9 por via oral ou enteral. No entanto, isso pode n\u00e3o ser poss\u00edvel por uma s\u00e9rie de fatores e, nesses casos, \u00e9 importante garantir a oferta de zinco pela via <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/vantagens-da-nutricao-parenteral-pronta-para-uso\/\">parenteral<\/a>.<\/p>\n<p>As recomenda\u00e7\u00f5es di\u00e1rias de zinco parenteral, no entanto, s\u00e3o diferentes da ingest\u00e3o diet\u00e9tica por alimenta\u00e7\u00e3o oral, pois o paciente em nutri\u00e7\u00e3o parenteral pode ter uma necessidade maior do elemento devido aos fatores listados no quadro anterior, como defici\u00eancia mineral pr\u00e9-existente, bem como presen\u00e7a de doen\u00e7a que aumente a perda ou demande maior consumo de zinco.<\/p>\n<p>A quantidade de zinco ofertada a cada paciente, portanto, varia de acordo com a avalia\u00e7\u00e3o do m\u00e9dico, que pode usar como refer\u00eancias as diferentes propostas que atualmente existem em debate, como listado a seguir.<\/p>\n<ul>\n<li><strong>American Gastroenterological Association<\/strong> &#8211; 2,5-4 mg (38-61 \u03bcmol).<sup>3<\/sup><\/li>\n<\/ul>\n<ul>\n<li>For\u00e7a-tarefa para a Revis\u00e3o de Pr\u00e1ticas de Seguran\u00e7a da Nutri\u00e7\u00e3o Parenteral da <strong>ASPEN<\/strong> \u2013 2,5-5 mg (trata-se de uma aproxima\u00e7\u00e3o).<sup>4<\/sup><\/li>\n<\/ul>\n<p>Poder\u00edamos citar recomenda\u00e7\u00f5es para outras condi\u00e7\u00f5es igual fiz na tabela sabe? Ou mesmo o comp\u00eandio da BRASPEN.<\/p>\n<ul>\n<li><strong>Outras recomenda\u00e7\u00f5es<\/strong>: aumentar a oferta de zinco proporcionalmente ao volume gastrointestinal perdido em pessoas com perdas fecais aumentadas:\n<ul>\n<li>Pacientes com diarreia, mas intestino delgado intacto, perdem 15,2 mg de Zn por litro de l\u00edquido ent\u00e9rico.<sup>5<\/sup><\/li>\n<li>Pacientes com s\u00edndrome do intestino curto perdem 3,6 mg de Zn por litro.<sup>5<\/sup><\/li>\n<li>12 mg de Zn devem ser adicionados por litro perdido na diarreia, estoma ou f\u00edstula.<sup>6<\/sup><\/li>\n<li>Pacientes que recebem nutri\u00e7\u00e3o parenteral domiciliar: 3 a 15 mg, a depender da presen\u00e7a ou n\u00e3o da s\u00edndrome do intestino curto<sup>7<\/sup><\/li>\n<li>Trauma ou sepse: adicional de 2 mg \/ dia durante o catabolismo agudo.<sup>5<\/sup><\/li>\n<li>Pacientes em risco de s\u00edndrome de realimenta\u00e7\u00e3o: dose de ataque de 10-30mg\/ dia, seguida pela dose de manuten\u00e7\u00e3o de 2,5-5mg\/ dia.<sup>8<\/sup><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<ul>\n<li>Em crian\u00e7as, as necessidades de Zn dependem da taxa de crescimento:\n<ul>\n<li>Beb\u00eas requerem mais Zn por quilo de peso corporal do que adultos.<\/li>\n<li>Beb\u00eas prematuros: 400 \u03bcg \/ kg \/ dia<sup>1,2<\/sup><\/li>\n<li>Beb\u00eas a termo (&lt; 3 meses de idade): 250 \u03bcg \/ kg \/ dia<sup>1,2<\/sup><\/li>\n<li>Beb\u00eas a termo (&gt; 3 meses de idade): 100 \u03bcg \/ kg \/ dia<sup>1,2<\/sup><\/li>\n<li>Crian\u00e7as: 50 \u03bcg \/ kg \/ dia (m\u00e1ximo, 5000 \u03bcg \/ dia).<sup>1<\/sup><\/li>\n<li>Quando a nutri\u00e7\u00e3o parenteral \u00e9 complementar em vez de total, ou quando sua dura\u00e7\u00e3o \u00e9 limitada a at\u00e9 4 semanas, o zinco \u00e9 o \u00fanico tra\u00e7o elemento que deve ser administrado. Outros oligoelementos n\u00e3o s\u00e3o considerados necess\u00e1rio at\u00e9 2 semanas de idade.
