Entretanto, uma parte apresenta complica\u00e7\u00f5es e precisa de hospitaliza\u00e7\u00e3o. De acordo com o<a href=\"https:\/\/66b28c71-9a36-4ddb-9739-12f146d519be.usrfiles.com\/ugd\/66b28c_6092444f9bf04a7f91e6d7a73cf7ce3c.pdf\" target=\"_blank\" rel=\"noopener noreferrer\"> Parecer Braspen\/Amib para o Enfrentamento da Covid-19 em Pacientes Hospitalizados,<\/a> <strong>cerca de 5% dos pacientes precisam de terapia intensiva.<\/strong> Neste subgrupo, as complica\u00e7\u00f5es mais frequentes s\u00e3o a disfun\u00e7\u00e3o respirat\u00f3ria e a disfun\u00e7\u00e3o renal.<\/p>\n<p>Esses quadros podem ser especialmente prejudicados em caso de desnutri\u00e7\u00e3o. Por isso, as institui\u00e7\u00f5es de sa\u00fade v\u00eam adotando protocolos para a decis\u00e3o sobre o uso de <a href=\"https:\/\/www.fresenius-kabi.com\/br\/produtos\/nutricao-enteral\">nutri\u00e7\u00e3o enteral<\/a>, <a href=\"https:\/\/www.fresenius-kabi.com\/br\/produtos\/nutricao-parenteral\">nutri\u00e7\u00e3o parenteral<\/a> e outras medidas ligadas \u00e0 nutri\u00e7\u00e3o cl\u00ednica no manejo da COVID-192;3. Confira neste texto quais s\u00e3o as necessidades nutricionais de pacientes com COVID-19.<\/p>\n<h4>Desnutri\u00e7\u00e3o \u00e9 sinal de alerta<\/h4>\n<p>De acordo com a <em><a href=\"https:\/\/aspenjournals.onlinelibrary.wiley.com\/doi\/full\/10.1002\/jpen.2036\" target=\"_blank\" rel=\"noopener noreferrer\">American Society for Parenteral and Enteral Nutrition<\/a><\/em> (Aspen), um levantamento demonstrou que a preval\u00eancia de desnutri\u00e7\u00e3o nos pacientes com COVID-19 na cidade onde a pandemia se iniciou, em Wuhan, China, foi de 52,7%, levando \u00e0 diminui\u00e7\u00e3o da fun\u00e7\u00e3o imunol\u00f3gica (inata e adaptativa) e diminui\u00e7\u00e3o da resist\u00eancia \u00e0 infec\u00e7\u00e3o<sup>3<\/sup>.<\/p>\n<p>O alerta \u00e9 importante pois, entre os pacientes hospitalizados com a doen\u00e7a, a<strong> desnutri\u00e7\u00e3o adquirida e a sarcopenia (perda de massa muscular) podem resultar em hospitaliza\u00e7\u00e3o prolongada,<\/strong> prejudicadas pela imobilidade e ventila\u00e7\u00e3o mec\u00e2nica<sup>3<\/sup>.<\/p>\n<p>J\u00e1 entre os pacientes cr\u00f4nicos, \u00e9 poss\u00edvel que haja sintomas gastrointestinais variados que podem limitar a ingest\u00e3o de alimentos e induzir problemas de nutri\u00e7\u00e3o devido a um processo inflamat\u00f3rio prolongado. Segundo a Aspen, esse processo pode exacerbar o catabolismo (respons\u00e1vel pela produ\u00e7\u00e3o de energia no organismo) e a anorexia (perda de apetite), agravando a desnutri\u00e7\u00e3o, impedindo a recupera\u00e7\u00e3o e levando \u00e0 perda de independ\u00eancia e \u00e0 redu\u00e7\u00e3o da qualidade de vida<sup>3<\/sup>.<\/p>\n<hr \/>\n<p><strong>Leia tamb\u00e9m: <\/strong><a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/glutamina-parenteral-quando-usar\/\">Glutamina parenteral: quando usar?<\/a><\/p>\n<hr \/>\n<h4>Pacientes em terapia intensiva (UTI)<\/h4>\n<p>Conforme a<a href=\"https:\/\/www.clinicalnutritionjournal.com\/action\/showPdf?pii=S0261-5614%2818%2932432-4\" target=\"_blank\" rel=\"noopener noreferrer\"> ESPEN 2019<\/a>, <strong>o paciente que permanece por mais de 48 horas na UTI deve ser considerado em risco de desnutri\u00e7\u00e3o<\/strong> e, portanto, se beneficia de terapia nutricional precoce e individualizada4. Estas s\u00e3o algumas das recomenda\u00e7\u00f5es da Braspen\/Amib para pacientes em terapia intensiva internados com COVID-19.<\/p>\n<h5>Via de alimenta\u00e7\u00e3o<\/h5>\n<ul>\n<li>A alimenta\u00e7\u00e3o por via oral \u00e9 a preferencial em pacientes n\u00e3o graves com diagn\u00f3stico de COVID-19, incluindo a utiliza\u00e7\u00e3o de suplementos orais e terapia nutricional enteral quando a ingest\u00e3o energ\u00e9tica estimada for menor que 60% das necessidades nutricionais.<\/li>\n<li>Em pacientes graves, a nutri\u00e7\u00e3o enteral \u00e9 a via preferencial e deve ser em at\u00e9 48 horas<sup>6<\/sup>.<\/li>\n<li>No caso de contraindica\u00e7\u00e3o da via oral e\/ou enteral, a nutri\u00e7\u00e3o parenteral deve ser iniciada o mais precocemente poss\u00edvel. Ela tamb\u00e9m deve ser considerada de forma suplementar ap\u00f3s 5 a 7 dias em pacientes que n\u00e3o conseguirem atingir aporte cal\u00f3rico proteico superior a 60% por via digestiva<sup>2<\/sup>.<\/li>\n<\/ul>\n<h5>Aporte cal\u00f3rico na fase aguda<\/h5>\n<ul>\n<li>Iniciar com um aporte mais baixo, entre 15 kcal\/kg\/dia e 20 kcal\/kg\/dia e progredir para 25 kcal\/kg\/dia ap\u00f3s o quarto dia ou ap\u00f3s fase aguda inicial.<\/li>\n<li>Utilizar f\u00f3rmulas enterais com alta densidade cal\u00f3rica em pacientes com disfun\u00e7\u00e3o respirat\u00f3ria aguda e\/ou renal.<\/li>\n<li>Pacientes gravemente doentes, mesmo em caso de disfun\u00e7\u00e3o renal aguda, devem receber oferta proteica entre 1,5 g\/kg\/dia e 2 g\/kg\/dia, a fim de evitar balan\u00e7o nitrogenado negativo. Em pacientes em terapia de substitui\u00e7\u00e3o renal cont\u00ednua, consideram-se at\u00e9 2,5 g\/kg\/dia<sup>2<\/sup>.<\/li>\n<\/ul>\n<hr \/>\n<p><strong>Leia tamb\u00e9m:<\/strong> <a href=\"https:\/\/lam.unitedforclinicalnutrition.com\/pt-br\/nutricao-parenteral-intradialitica\/\">Nutri\u00e7\u00e3o parenteral intradial\u00edtica (npid) contribuindo com a melhora do estado nutricional<\/a><\/p>\n<hr \/>\n<h4>Pacientes com c\u00e2ncer<\/h4>\n<p><strong>Os pacientes com c\u00e2ncer fazem parte do grupo de risco para COVID-19<\/strong> por apresentarem diminui\u00e7\u00e3o da imunidade, como resultado da pr\u00f3pria doen\u00e7a ou do tratamento com imunossupressores e outros m\u00e9todos. Al\u00e9m disso, tendem a ter a ingest\u00e3o alimentar prejudicada, o que demanda vigil\u00e2ncia constante.
